2: You need the "MythicLich_Friends_HasNoFriends" playing. You get this by choosing to kill both advisors during the Act 5 quest where they are fighting. I believe you have to first support one, kill the other, and then turn on the first and kill them. I didn't get a chance to test whether you can just start the etude or not.

The lack of ability to rez undead DOES become an issue later though if you go with a Lich companion party. There are places likely to kill a party member where you cannot rest.
