Multi-hit simply makes it so nearly every single melee weapon can hit up to 3 zeds at the same time. Basically every melee weapon except small knives or pens/forks and such. I believe cleavers are the exception among those.

Multi-Hit is a sandbox option: Character > Weapon Multi-hit. Aim outline is a game setting: Options > Cursor > Aim Outline > "Any Weapon". If you play Apocalypse, Multi-Hit is disabled by default. Aim Outline is disabled by default on any mode since it's an option setting.
